Train as peer support

Train as peer support

For a senior experiencing loss, isolation, health challenges, a change in housing or other stressors, knowing that someone cares can make a world of difference. Jewish Seniors Alliance’s Peer Support Services trains people aged 55+ to be that someone – seniors helping seniors.

JSA offers at least two peer support training sessions a year, and the next one begins Sept. 20. The training, conducted over Zoom, is free. It comprises 11 sessions, or 44 hours. Participants will learn active listening and other communication skills, how to set boundaries, about aging and the health issues that can accompany it, about available community resources, and more. Upon successfully completing the course, volunteers (who are required to pass a criminal records check) will receive a certificate from Senior Peer Counseling of B.C. and be matched with a senior in the community.

The Jewish Independent spoke with Grace Hann, JSA senior peer support services trainer and supervisor, and co-trainer Miguel Méndez about the program.

JI: When did the peer support program start, and how has its effectiveness been measured?

JSA: The peer support program started in 2011, inspired by [JSA president emeritus] Serge Haber’s vision to support vulnerable seniors in the community. We gauge the program’s effectiveness through detailed statistical analysis. This includes client satisfaction surveys administered after six months, then again after one year, and annually thereafter.

JI: What qualities do your most successful volunteers possess?

JSA: Key qualities for a thriving volunteer-client relationship are empathy, patience and compassion. It’s also crucial for volunteers to be receptive to new ideas during training, which enhances their understanding of the challenges many seniors face.

JI: What is the biggest challenge you face while training volunteers?

JSA: Most volunteers join our training eager to learn and contribute. The training refines their listening skills, helps them establish healthy boundaries, and fosters an understanding of underlying issues many seniors confront, such as grief, loss, and the challenges of connecting with new communities.

JI: What surprises you most about your day-to-day work?

JSA: We’re continually surprised by the deficiencies in our medical and social support system. Many case managers from various health units emphasize their shortage of time and personnel to provide adequate emotional support for seniors. Heart-wrenching sentiments from vulnerable seniors, like “I no longer matter in society” or “My friends have all died,” often resonate with us.

JI: What are the most significant changes you have noticed in the needs of seniors over the last five years?

JSA: There’s a growing mental health support gap for seniors. Many are grappling with conditions like anxiety and depression. At JSA, we receive increased requests from professionals seeking support for such individuals.

However, our volunteers are not mental health experts. While we acknowledge the need, we must be cognizant of our role and limits as volunteers. Additionally, many seniors are anxious due to rising housing and grocery costs, severely affecting those with limited incomes.

JI: How has your training program adapted post-COVID?

JSA: During the pandemic, we transitioned our training to a digital platform, Zoom, allowing many seniors to engage without leaving their homes. We’ve maintained this digital approach, which now attracts participants from a broader geographical range.

JI: Is there a piece of advice or aspect of training that you consider most impactful to your volunteers?

JSA: Role-playing is an exceptionally effective tool. It elicits profound feelings and introspection, heightening volunteers’ empathy and allowing them to better understand others’ experiences. Since our training is experiential, volunteers gain by sharing their own wisdom and experiences.

JI: Anything else you’d like to add?

JSA: Conducting initial client intakes can be deeply emotional. For instance, a recent case involved a 91-year-old woman, previously independent, whose life altered drastically after a spine fracture from a fall. With no family and deceased friends, she faced a significant emotional void. While we can’t mend her physical injuries, we can offer invaluable emotional support, offering hope and the comfort of knowing someone cares. This not only benefits her but also gives our volunteers a sense of purpose and the gratifying feeling that comes from assisting others.

Emotional support vital

On Nov. 30, as part of the Fraser Health Virtual Geriatric Educational Session entitled The Importance of Emotional Support for Seniors, Grace Hann, Jewish Seniors Alliance peer support services supervisor and trainer of volunteers, made a presentation that featured recorded remarks from JSA president emeritus Serge Haber.

Haber pointed out in his remarks how life changes for many older seniors once they retire. They often cease to be valued and become invisible, he said. The JSA’s Peer Support Services (PSS) program has trained seniors to provide emotional support to other seniors – active and reflective listening, encouraging the senior to talk about their issues and finding solutions on their own, but with support.

When Haber took the training course, he learned how crucial the PSS program is for the well-being of seniors. The support provided helps them deal with tremendous changes in their lives, such as loss of family, loss of position in society and health issues. Haber argued that these needs are not usually recognized. The gains made by the clients of PSS, he said, are phenomenal.

Hann pointed out that the training and volunteering also helps the seniors who become volunteers.

The second half of the presentation consisted of an explanation by Hann of the training process and a description of the PSS program, as well as other JSA activities, including education and advocacy. Charles Leibovitch, PSS coordinator and the social worker for the program, spoke about Haber’s passion for the work they are doing and how his passion has inspired many of the staff and the volunteers.

Older seniors can remain alone at home longer, if they would like to, as a result of the government’s Better at Home program. However, there is little in emotional support offered; it is not just a gap in this area, but a chasm.

Alvarez thanked everyone and mentioned the summit Fraser Health is planning in June for further discussion of these topics.

Peer service reinvented

To say that COVID-19 has wreaked havoc on British Columbians would be an understatement. The virus has disrupted anything that we would call the normal activities of daily life. Most of us have had to make major compromises: where we go, what we do and how we can avoid getting the terrible virus. These adjustments have had a major impact on Jewish Seniors Alliance clients, who are already compromised by loneliness and isolation.

How we serve our clients and how we support our volunteers has undergone major changes – let’s give it the label “reinventing peer services.” In order to better understand what has taken place, I interviewed Charles Leibovitch, JSA senior peer support services coordinator, and Grace Hann, JSA trainer of volunteers of senior support services.

In the beginning

By mid-March 2020, the first sign of COVID-19 began to show its ugly head. The lockdown left clients and volunteers absolutely unprepared. Persons who were already isolated and lonely found themselves even more isolated and lonelier. As time progressed, clients were cut off from family members who might have supported their relatives through personal contact and social events. This was especially devastating for persons without family.

In many situations, volunteers were their primary contact; their lifeline! Being alone undermines one’s mental health. Being alone exaggerates one’s fear of COVID-19. Most of the clients were cut off from community programs, like adult day care. Spouses who usually spent time with their spouse in a long-term care facility were also cut off. Simple activities like going for a walk and sitting on a bench were curtailed. Elders had depended on having that human connection – having that human touch makes us feel needed and whole.

Volunteers meet challenge

The changing scene called for quick action, initiated by Grace and Charles. Instead of personal visits, the telephone would become the prime instrument of contact between volunteers and their clients. It was necessary to contact the volunteers quickly. Support for the volunteers would be provided by Zoom. This necessitated a steep learning curve for volunteer and client. After all, making and keeping the connection was critical. The three services – peer support, friendly visits and friendly phone calls – had to be reassessed in terms of the neediest clients. Each of the three services’ volunteers had different levels of training by Grace.

In some situations, a certified peer support volunteer was assigned to a person who ordinarily would have had contact with a friendly visitor or a friendly phone caller. Moving from in-person contact to impersonal contact was a major transition – almost like reinventing how support was to be provided. The JSA volunteers made the transition like veterans, with the extraordinary help of Grace and Charles. There was an increase in the contacts between volunteers and clients and an increase in Zoom online meetings to support the very special work being carried out by the volunteers.

Supporting the volunteers

Grace and Charles organized many activities, including outdoor picnics, weekly webinar seminars, a Chanukah party with a singalong and group support meetings every three weeks. The spirit and esprit de corps by the volunteers has been amazing. Volunteers will send cards to their clients as an additional way to keep in contact. Who doesn’t like to receive mail?

Next steps

Challenging times require challenging solutions. Charles and Grace rose to the challenge and proved that, with dedication, imagination and determination, obstacles can be overcome. When the COVID-19 vaccine has been fully distributed, we will establish a “new normal.” This will present JSA, Grace, Charles and the volunteers with a new set of issues and situations. And, as the song goes, “we shall overcome” – they will face these challenges with creativity, empathy and caring.

A capacity crowd of 175 gathered at the Peretz Centre for Secular Jewish Culture on Sunday, Oct. 26, to learn more about mental health and wellness, the topic of this year’s Jewish Seniors Alliance of Greater Vancouver fall symposium.

JSA president Marilyn Berger opened the session by asking the audience to rise in the name of mental health and to honor the soldiers who had lost their lives that week in Canada. This was followed by O Canada, led by Barbara Bronstein and Debbie Cossover, with Claire Cohen joining them in the singing of Hatikvah, and Marshall Berger accompanying on piano.

Gyda Chud introduced herself and co-convener Bev Cooper, and proceeded to inform those in attendance of the various projects that are JSA’s main concerns: advocacy, Senior Line Magazine, peer support counseling and the Empowerment Series. She went on to explain that the day’s topic had been chosen by attendees at past events, via the evaluation cards they had filled out citing this issue as a particular interest.

Cooper introduced the first speaker and the panel moderator, Dr. Penny MacCourt, past president of the B.C. Psychogeriatric Association, who admitted that she, too, will become a senior this summer.

MacCourt said that mental health is often equated with mental illness but that they are not the same thing. She emphasized the need to teach people ways in which to cope in the face of adversity; to help moderate the impact of stress, and facilitate social and emotional well-being. We as a society need to provide supportive living shelters, and continuous inclusiveness and access to such services, she said.

Help should be provided to those in need to maintain self-esteem and achieve effective coping strategies, she added, as these are the “protective factors” that can ease or ward off risks, including social isolation, limited income, loneliness, challenging life transitions, and lack of meaningful activity.

Dr. Martha Donnelly spoke next. At one time the director of the division of geriatric psychiatry at the University of British Columbia and a leader in the development of guidelines for geriatric mental health practice, she outlined some “red flag” symptoms of depression: appetite disturbance, loss of weight, early morning wakefulness, lower energy, and wishing life were over. The highest rate of suicide is in the senior years, she said.

One of Donnelly’s patients, Don Carroll, a very young 82, offered the audience a glimpse into his former work life, which included being an instrumental part of TV shows such as Mr. Dress Up and The Friendly Giant, and his subsequent descent into a depression from which he could not emerge on his own. With Donnelly’s continuing help, Carroll has returned to being an outgoing, fun, contributing person; a difficult journey for him, his supportive wife, Nancy, and family. It took resolute determination on all their parts to get Carroll to where he is now – off any medication and sharing with others his belief that one can heal with the correct diagnosis, therapy, doctor and support, to regain the ability to rely on oneself. The process was slow, including group therapy, daily exercise, medication and “thought catching,” tossing out negative thoughts before they take hold.

Grace Hann, who is currently working with JSA as a trainer and supervisor of peer support services, acknowledged JSA president emeritus Serge Haber for his vision to initiate the peer services as a vital project of JSA.

Hann is president of Senior Peer Counseling of British Columbia and on the YWCA board of the Community Action on Elder Abuse Project. She explained that it takes a peer to fully comprehend the feelings one is experiencing, such as loss of a loved one, age-related challenges, relocation, family discord – all situations that need empathy, which she described as “echoes of another person in ourselves.”

Hann called upon three graduates of JSA’s program to do role-playing, one of the methods used in the 55-hour course in peer counseling. They performed skits depicting examples of exchanges between clients and counselors at Week 1 and in Week 54. It showed the process through which the trainees had gone and from which they had grown from the initial expectations of their own abilities and finally gaining the knowledge and understanding of how to deal with the challenges clients face, such as loss of vision, a loved one and/or freedom and independence.

Trainees are taught to not use JAR: judgment, advice or rescue. Rather, counselors employ the three Es: empowerment, empathy and emotion. Both clients and counselors have benefited from the interactive sessions, said Hann, noting that there is a waiting list.

Hann then introduced a special guest, Tanja, 91, who, when Denmark was invaded by the Germans, secretly and at high risk to herself, helped Jewish adults and children escape to Sweden. Tanja shared that one of her most gratifying moments was witnessing the uniting of a mother with her child in a kindergarten when the war was over. She also shared that, many years later, here in Canada, when she was ill with cancer and reached out for help, she received empathy and understanding. Tanja was given a standing ovation by those attending, many of whom had been moved to tears listening to her.

The symposium came to a close with Berger thanking the speakers, presenting them with gift certificates. She made special mention of the co-conveners as well as JSA coordinator Karon Shear and the entire symposium committee for putting together such a successful event.

Refreshments and discussions followed. The audience left with much to contemplate but assured in the knowledge of where and to whom to turn should the need arise.
